- Patients are referred to Priority Care by their primary care provider, a specialist, or after a hospitalization. Once enrolled, they transition to being managed by our Priority Care team rather than their PCP.
- The program includes frequent provider visits, comprehensive case management, and access to additional resources such as pharmacy support, community resource advisors, and hospice coordination as needed.
- As a physician in the Priority Care Department, you will play a critical role in stabilizing and educating patients on their conditions, with the ultimate goal of transitioning them back to their primary care provider with the knowledge and tools needed to manage their health effectively.
- You will work alongside a dedicated team, including nurse case managers who maintain regular contact with patients, ensuring continuity of care between visits.
